5) CanTrustRx Inc. will only disclose personal information for the purpose of which it is collected or if the customer has consented to the disclosure of their personal information, or when it is deemed necessary or applicable by law.

6) CanTrustRx Inc. is responsible for the maintenance and accuracy of personal information. It is our responsibility to ensure all record of personal information is presented in a clear, up to date format necessary to accomplish the purpose for which the information has been collected

7) CanTrustRx Inc. is responsible for ensuring that the personal information collected is protected to an appropriate standard in accordance to the level of sensitivity associated with the information

8) CanTrustRx Inc. will make information accessible to our customers concerning the policies associated to the management of their information

9) CanTrustRx Inc. will inform the customer of the use and/or divulgation of their personal information upon their request. Customers may verify the accuracy of their personal information, and amendments may be requested if deemed necessary.

10) Customers may speak with a pharmacist regarding any questions, complaints, enquires, and/or suggestions the may have in regards to the information outlined above.

Collection of personal information

CanTrustRx Inc. collects personal information required to fill your request for our products and/or services. The personal we collect includes (but is not limited to) the following:

First and last name, telephone number, mailing address, date of birth, age, weight, height, sex, medical history (such as allergies, previous or current medical conditions), referral source, primary physician, the contact information (such as address, phone number) of your physician, the medication you are requesting, medications prescribed to you presently and previously, credit card information (including card holder’s name, mailing address, credit card type, credit card number, expiration date), prescription information, etc.

It is the customers’ decision to provide personal information to CanTrustRx Inc. If the customer decides to withhold important details, this may limit the products and/or services we will be able to provide to you. A choice to withhold information by the customer may result in difficulty to provide proper advisement and/or suggestions. Please note that any anonymous and/or non personal information collected cannot be traced back to a specific individual or employee.

Use of Personal Information

We use your personal information as a means of communication with the customer of whom it belongs to and the fulfillment of prescription orders. We are required to provide your information to a Canadian physician for the purpose of reviewing your prescription order. We only use the personal information that you have provided to use for the reasons previously mentioned. If there is a reason to disclose your information for an alternate reason, we will inform the customer and obtain consent before proceeding with the divulgation of such information. If legally permitted or deemed legally necessary, we may be required to disclose your information to the necessary authorities. In this instance, we will insure to ensure that the authorities have the necessary grounds to obtain such information, and we will only provide the specific information requested.
Please note that when you visit our website, the personal information you submit to CanTrustRx Inc. is submitted on a voluntary basis. It is your option to view our website at anytime without providing personal information.

Security

We have security measures in place to ensure that your personal information is not shared or viewed by unauthorized personnel. Your personal information is protected against misuse of information provided, unauthorized access, and disclosure of information provided. Files generated electronically are kept in a secure setting and all paper files are kept in restricted access areas.
